To use IPv6 privacy extensions globally:

/etc/NetworkManager/conf.d/ip6-privacy.conf

[connection]
ipv6.ip6-privacy=2

The uppercase directives refer to the active connection being used. Whereas the lowercase refers to the connection profile, so it's specific to what connection you configure and at some point activate.
